Reservation Procedure

Guardians of consumers wishing to participate in an Off The Beaten Path trip are required to read the reservation and cancellation policies, the medication policies, and the behavior and emergency policies.

The guardian must also complete the following forms:

• Emergency Medical Care Authorization Form

• Reservation Policies and Contract

• Video Release Form
